Once you are given a surgical date by your surgeon or proceduralist, Dr Wan asks that you fill in the pre-operative questionnaire. This is so that your anaesthetic will be as safe and seamless as possible.
This questionnaire will ask you about your past medical history, any medications that you are currently on, as well as any other information that may be relevant to your upcoming anaesthetic.,

Based on the information from your questionnaire, Dr Wan will contact you if further information is required. Dr Wan will also give you advice regarding medications and fasting based on this information. Dr Wan will also answer any questions regarding your upcoming anaesthetic as well.
